State the observed result
Write what happened in one sentence, including the visible state and the action that led there.

Capture the target
Select only the relevant region or display. Include enough surrounding UI to identify where the problem lives.

Add one arrow, box, or short label. Keep the annotation outside the control when possible so the original pixels remain legible.

Describe expected behavior
Say what should have happened and include a build, route, or reproduction detail when it changes the interpretation.
